Μετά την κυκλοφορία του ομώνυμου ντεμπούτου τους (το οποίο όρισε ουσιαστικά ένα νέο μουσικό ιδίωμα) οι Black Sabbath ηχογράφησαν (την ίδια χρονιά) στα "Regent Sound Studios" του Λονδίνου τον διάδοχό του υπό τον τίτλο "War Pigs".

Η δισκογραφική εταιρεία τους ζήτησε ένα επιπλέον τραγούδι με τους Sabbs να μην έχουν κάποιο έτοιμο πέραν του "Paranoid", το οποίο δεν ήταν και από τα... αγαπημένα τους! Τελικά όχι μόνο μπήκε στον δίσκο, όχι μόνο "βάφτισε" τον δίσκο, αλλά έγινε και το πιο επιτυχημένο και αναγνωρίσιμο τραγούδι τους.

Ο (μπασίστας της μπάντας) Geezer Butler είχε δηλώσει τον Μάρτιο του 2004 στο περιοδικό Guitar World σχετικά με το τραγούδι:

«A lot of the Paranoid album was written around the time of our first album, Black Sabbath. We recorded the whole thing in about 2 or 3 days, live in the studio. The song "Paranoid" was written as an afterthought. We basically needed a 3 minute filler for the album, and Tony came up with the riff. I quickly did the lyrics, and Ozzy was reading them as he was singing.»
